JOB PURPOSE
Responsible to serve as an Advanced Practice Provider for Medication Assisted Therapy (MAT), responsible for conducting assessments and treatment planning for patients suffering from behavioral health or substance use disorders. The APP-MAT will evaluate Opioid Use Disorder patients for our remote tele-MAT program involving the use of Buprenorphine and other supportive measures.

Here are our MUST HAVES:

A Master’s degree as a Physician Assistant or Nurse Practitioner, and at least three to five years of prior
work experience in clinical setting (Psych NP preferred, ideally having completed training to administer
Buprenorphine under waiver).

Experience Specifically Required

  • Buprenorphine Waiver training, ideally allowing management of up to 275 patients/month
  • Nurse Practitioner or Physician Assistant, ideally with Psych or OUD/MAT experience

    CERTIFICATES, LICENSES, REGISTRATIONS, PROFESSIONAL DESIGNATIONS

    • A valid APRN or PA license and must maintain their license for the defined geographic location of service (Kentucky). Required
    • DATA 2000 waiver (approved training) to prescribe Buprenorphine. Required

    You will be responsible to:

    • Implements the national standards of healthcare as adopted by the Company for telemedicine, behavioral health, and OUD; ensures the best experience possible for both patients and providers.
    • Meets with an assigned patients via a scheduled, telehealth consultation to conduct a history and physical, which includes a review of medical, psychiatric, and surgical record, medications, substance abuse history and allergies.
    • Develops a thorough list of the patient’s chronic conditions to establish the individual’s eligibility to participate in our "Primary Addiction Treatment at Home" (PATH) Program for Opioid Use Disorder (OUD).
    • Orders the services and medication(s) required for those patients who are both eligible to receive services and willing to participate in the program. These medications include Buprenorphine, prescribed under their DATA 2000 waiver.
    • Documents telehealth consultations upon completion of their initial assessment as well as periodic medical provider check-ins and ensures the proper CPT and ICD-10 codes are identified for medical billing by our internal staff.
    • Utilizes our proprietary telehealth system and dashboard to interact with patients; reviews patient survey results and may monitor vital signs in certain cases; follows processes and reacts to changes in survey responses, overall patient status, vital sign thresholds, alerts, and triage stages.
    • Reviews and evaluates patient behavior and tele readings; provides proactive education and health and wellness support as needed for individuals suffering from one or more serious medical conditions.
    • Coordinates care with the PATH team, under the supervision of the Kentucky Behavioral Health Director, to include licensed Addiction Counselors, Behavioral Health Coaches, and others as appropriate.
    • Refers patients to a higher level of care (in-patient, acute emergency care or other) as appropriate.
    • Telemonitors remote collection of saliva-based drug screens on a periodic basis.
    • Participate on bi-weekly update meetings with the Kentucky Behavioral Health Director or at other times as deemed necessary.
    • Performs all duties on an agreed-upon schedule as assigned by supervisor.
    • Participates in ongoing, internal, and external training to improve health coaching skills.
